Camas High School Athletic Program
A current physical within the past two (2) years, the concussion form, and the athletic medical emergency form need to be submitted before an athlete can begin practice. Please keep in mind that athletes need to turn that paperwork into the Athletic Office prior to their practice time and not show up at practice with the forms for the coach.
Camas High School has carried a tradition of excellence in athletics for many years. There are many many accomplishments that the Camas High School sports program can be proud of.
Sports can play a big role in the development of a student athlete's life skills. Sports teaches discipline, team work, social skills, and keeps our youth healthy and active. An athlete must have a current physical (good for two years from the date of the exam), a concussion form, a medical emergency form, and must have purchased his/her ASB card and paid the athletic fee before the first practice. Some sports have try-outs for the team and in that case the ASB card and fee must be paid when the athlete makes the team.
In order for the athlete to self-carry and self-administer medication for asthma or severe allergies the Authorization for Administration of Medication Form must be completed by the licensed health care provider and the parent/legal guardian.
All completed forms are to be turned in a the Athlete Office.
